Clinical History
The patient is a 72-year-old man with a 30 year history progressive, symmetric leg weakness and tingling. Past medical history includes arthritis, diabetes, and skin cancer. Medications include diclofenac, atorvastatin, and amlodipine .
Physical exam shows Normal bulk and tone and 3/5 strength in proximal lower extremities. EMG showed severe chronic active sensory greater than motor axonal neuropathy, a C5 radiculopathy and a proximal irritable myopathy.
What is your diagnosis?
Mitochondrial myopathy
- Mitochondrial dysfunction may result from both genetic and toxic etiologies.
- Drugs, such as statins can precipitate mitochondrial dysfunction in predisposed individuals (i.e. those with occult or subclinical mitochondrial disorders).
- Genetic testing is indicated.
